As of January 1, 2020, Medicare updated the reimbursement for remote physiological monitoring (RPM).

It wasn’t very long ago when the typical practice was full of activity. Patients were calling and appearing in person to be examined for a myriad of conditions. Surgeries were being performed. Therapy was being prescribed and patients were being seen for all sorts of conditions. Little thought was given to managing your practice without personal contact with patients.

Suddenly, a national health crisis was declared and “nonessential services” were halted and patients asked to stay home. Outside of trauma injuries, the steady flow of patients became a trickle. The carefully crafted business of medicine found itself in the same predicament as the local restaurant. There is no income without consumers. Without patients there is no means to pay employees. Like many small businesses, practices are either going to close or find new ways to take care of and interact with their patients.

Sometimes it takes a crisis to get physicians to examine a new way to manage patients. While we were busy running customer centric businesses, the Centers for Medicare and Medicaid (CMS) was quietly moving towards the world of remote patient monitoring and telehealth visits by developing reimbursable codes for healthcare personnel to use in the care of patients. In the first few years the rules were very restrictive but by January 2020 there were monthly codes available for remote physiologic monitoring of patients independent of face to face visits for chronic patient care.

What the recent national health care emergency did was highlight the availability of these codes and removed all restrictions. In response to “stay at home” we still have a responsibility to manage the long term health of our patients. Using this time to develop the office routines needed to take advantage of this evolving method of patient care can lead to a new steady ancillary income stream and assist in managing a greater number of patients with less overcrowding of offices. The aggregate of applicable codes could net up to $200 per patient per month.

Innovative uses of telehealth and telemedicine

“Under this new waiver, Medicare can pay for office, hospital, and other visits furnished via telehealth across the country and including in patient’s places of residence starting March 6, 2020. A range of providers, such as doctors, nurse practitioners, clinical psychologists, licensed clinical social workers and now physical therapists are able to offer telehealth to their patients. Additionally, the HHS Office of Inspector General (OIG) is providing flexibility for healthcare providers to reduce or waive cost-sharing for telehealth visits paid by federal healthcare programs announced in this bulletin, https://oig.hhs.gov/fraud/docs/alertsandbulletins/2020/policy-telehealth-2020.pdf

Telehealth, telemedicine, and related terms generally refer to the exchange of medical information from one site to another through electronic communication to improve a patient’s health. Innovative uses of this kind of technology in the provision of healthcare is increasing. Steprite™

Steprite™ is a medical device for remote/home monitoring which affords medical care prescribers the ability to customize physical rehabilitation protocols to meet each patient’s unique needs. In the event of a fall, MyNotifi® sends a text to friends, family members, loved ones, informing them whether or not the person using MyNotifi® is conscious.
